-Download the form template, print it, and bring it to a hospital for the required medical tests. The hospital will complete the form with your results.
-The medical examination must cover all items listed on the form.
-Ensure your photo is attached and the form is stamped and signed by the hospital after the examination.
-The medical results are valid for 6 months only.
-Forms that are incomplete, lack a final physician’s suggestion, missing the attending physician’s signature, official hospital stamp, or applicant’s sealed photo will be considered invalid.
-The document must be in English or Chinese.

Transcripts are a record of your academic accomplishments, including every course you took, when you took each course, and the grades you received.

13. Other materials

Applicants who have either one or both parents possess Chinese citizenship but are long-term/permanent residents in another country were already in possession of foreign citizenship by the time of birth, and cannot possess Chinese citizenship. This type of applicant should provide the following documents:
Past ID information of both parents during the applicant’s time of birth in photocopy form
Current ID information of both parents in photocopy form
Residency documents of one or both parents at the time of the applicant’s birth
The applicant’s birth certificate that proves non-Chinese nationality or acquisition of foreign citizenship

14. Other supporting documents

Applicants who have either one or both parents possess Chinese citizenship but are long-term/permanent residents in China were already in possession of foreign citizenship by the time of birth. This type of applicant should provide the following documents:
past ID information of both parents during the applicant’s time of birth in photocopy form
current ID information of both parents in photocopy form
Documents certifying the applicant forfeited Chinese citizenship for at least 4 (four) years
applicant’s birth certificate that proves non-Chinese nationality or acquisition of foreign citizenship

15. Others

Applicants who were previously Chinese nationals but have acquired citizenship from another country need to provide:
Documents certifying that the applicant has successfully acquired foreign citizenship
Documents certifying the applicant forfeited Chinese citizenship for at least 4 (four) years
Deactivation of Chinese citizenship that was previously in possession

*16. CSCA Test Transcript(s)

-You need to take the official CSCA test organized by the Chinese government and submit the corresponding subject result transcript(s) when applying. Learn more about the CSCA test >>
-The test covers Chinese (Humanities and STEM Chinese), Mathematics, Physics, and Chemistry. Mathematics is required for all programs, while other subjects depend on the university’s specific requirements.
-We are updating detailed information for each university, including the required subjects you have to take for each program and their score requirements. Once the updates are ready, you can check the attachment and information here for this university for the latest information. If no attachment is available yet, it usually means the university has not announced further details.

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
